The purpose of the IDD Transition Support
Team is to provide
support activities to the Local IDD Authorities (LIDDAs) and HCS and
TXHmL Providers that serve individuals with intellectual and
developmental disabilities (IDD) at risk of being admitted into an
institution, and those who have moved from institutional settings,
including state supported living centers (SSLCs) and nursing facilities
IDD Transition Support Team
can be requested by using the Transition Support Team Referral Form
- Training. Educational
activities and materials, such as webinars, videos, and other
correspondence, focused on increasing the expertise of LIDDAs and
providers in serving this population, will be provided at least
- Technical Assistance.
Technical assistance will be available upon request
from LIDDAs and program providers on specific conditions, with examples
of best practices and evidence-based services for individuals with
significant challenges that are at risk of being institutionalized.
- Case and Peer Review.
Case and peer review support will be provided to service
planning teams that need assistance providing effective care in the
transition for an individual with significant challenges.

Victoria, Wilson, and Zavala.
