Purpose

The purpose of the IDD Transition Support Team is to provide support activities to the Local IDD Authorities (LIDDAs) and HCS and TXHmL Providers that serve individuals with intellectual and developmental disabilities (IDD) at risk of being admitted into an institution, and those who have moved from institutional settings, including state supported living centers (SSLCs) and nursing facilities (NFs).

IDD Transition Support Team Services include:

  • Training.  Educational activities and materials, such as webinars, videos, and other correspondence, focused on increasing the expertise of LIDDAs and providers in serving this population, will be provided at least quarterly.
